How Much Does Outsourced Medical Billing Cost in California?
Updated: Oct 9
If you are comparing medical billing companies in California, the percentage quoted is only one part of the cost. The more important question is what your practice receives for that fee, how consistently claims are followed through to payment, and how much work remains with your staff.
How are outsourced medical billing services priced?
Many medical billing companies charge a percentage of collections, while others use a flat monthly fee, per-claim charge or a hybrid model. The right comparison depends on specialty, monthly collections, claim complexity, provider count and the services included. Ask each vendor to specify the exact revenue used to calculate its fee, whether patient payments are included, and whether minimum charges apply.
What can affect the cost?
Specialties with complex authorizations, procedure coding, workers compensation, out-of-network claims or high denial volumes may require more hands-on follow-up. A small practice with modest collections can also have a higher effective billing cost if a vendor charges a minimum monthly fee.
Before comparing quotes, confirm whether charge entry, claim submission, payment posting, patient statements, insurance AR follow-up, appeals, underpayment review, credentialing and reporting are included. Some vendors price these services separately.
A simple example of percentage-based billing
For illustration only, a 5% billing fee on $50,000 in monthly collections would equal $2,500. An 8% fee on the same collections would equal $4,000. Those figures are mathematical examples, not a quote or claim about typical California market rates. The actual value depends on services, collections performance and the work the practice must still perform.
Five questions to ask before signing
1. What exactly counts as collections for fee calculations? Are refunds, capitation, patient payments or old balances handled differently?
2. Who owns denied claims and aging receivables? Ask how follow-up, corrected claims, appeals and unresolved balances are tracked.
3. What reporting will we receive? Look for clear collections, adjustments, payer trends, denial reasons and actionable AR by age.
4. Can the billing team work with our current software? An unnecessary platform change can create training and transition costs.
5. What are the contract terms? Check setup fees, monthly minimums, termination provisions, data access and transition support.
Is the cheapest billing company the best choice?
Not necessarily. A lower fee is less valuable if follow-up is incomplete, payments are posted incorrectly or underpayments go unnoticed. Likewise, a higher fee is not automatically justified. Compare what each vendor actually does and ask how its processes protect collectible revenue.
